Authorization Public Release

Public Release: I authorize the use of the student's (name) and photo image associated with promotion of the Colorado Institute of Flamenco (CIF) and understand that the said materials are property of CIF to be used at the sole discretion of CIF including, but not limited to, print media, adverting, press releases, feature media/articles, radio and television news or promotion and social media. Liability Release

I hereby grant the permission necessary to participate in the Colorado Institute of flamenco (CIF) classes, performances, and activities. I, on my own behalf and on behalf of the student further agree to release and hold harmless CIF from an and all liability arising out of or connected with CIF classes, performances, and activities. This includes any claim arising or connected with any illness or injuring (minimal, serious, catastrophic, and/or death) that the participant may incur or sustain during CIF classes performances or activities. HEALTH & SAFETY
